A channel readback error occurs when the actual state of an output channel, as read back by the module, does not match the commanded state. This can be due to a wiring issue, an external load problem, or an internal fault within the I/O module, leading to an unreliable output.

1. Measure the voltage at the output terminal (e.g., Q1) when the output is commanded ON and OFF.
2. De-energize the circuit and measure the resistance or impedance of the external load (e.g., contactor coil) directly.
3. Check the continuity of the wiring from the output module to the external load.
4. Temporarily connect a known good and appropriate load (e.g., a small lamp) to the output terminal and re-test.
5. If external wiring and load are confirmed functional, replace the safety output I/O module.
