The connected battery's voltage has exceeded the maximum safe limit during or after a charging cycle. This is a critical condition that can lead to excessive gassing, overheating, and potential irreversible damage to the battery if not promptly addressed.

Immediately disconnect the battery from the charger to prevent further overvoltage and potential damage.
Verify the charger's output voltage regulation by measuring its open-circuit voltage (if safe to do so) or output voltage with a dummy load.
Check the charger's configuration parameters to ensure the correct battery nominal voltage and appropriate charge profile are selected for the connected battery type.
