For the ATV310 drive, this fault is detected when one phase on the drive's output to the motor is lost. This can be caused by poor electrical connections, damaged motor cables, or issues with a downstream contactor.

Check all power connections between the ATV310 drive output terminals (U, V, W) and the motor terminals for tightness and integrity.
Inspect the motor power cables for any signs of physical damage, nicks, or breaks in insulation.
If a downstream contactor is used, verify its wiring, contact integrity, and proper operation during run conditions.
Measure the voltage at the drive output terminals (phase-to-phase) with the drive enabled to confirm all three phases are present and balanced.
