One or more phases of the three-phase AC mains input power supply have failed or are missing. This leads to an unbalanced or insufficient supply voltage for the drive's rectifier and DC bus. Operating with a phase failure can cause excessive ripple on the DC bus, overheating, and damage to the drive components.

Measure the three-phase AC supply voltage (L1-L2, L2-L3, L3-L1) at the drive's input terminals to confirm balanced and nominal values.
Inspect the main power cabling and connections from the upstream power source to the drive's input terminals for damage, looseness, or disconnections.
Check all upstream protective devices (circuit breakers, fuses, main contactors) for proper operation, tripped status, or signs of failure, ensuring all phases are correctly supplied.
Verify the drive's input power parameters (e.g., nominal voltage, phase monitoring thresholds) match the actual mains supply and application requirements.
